Beneficiary Change Form (PDF) ā Select, change and add beneficiaries to the Plan.
Change of Name/Address (PDF) ā Update your mailing address, phone number and legal name.
Automated Clearing House (ACH) Request (PDF) ā Add or change your checking or savings information for automatic deposits to your bank account

Age 50 and Over Catch-Up (PDF) ā Participate in the Age 50 and over Catch-Up Provision. Learn more about catch-up options in the Learning Center.
Qualified Military Service Catch-Up (PDF) ā Explains the Planās Qualified Military Service Catch-Up provision and how to take advantage of it. To enroll in the Qualified Military Service Catch-Up, you must meet with your Account Executive.

Retirement Service Credit (PDF) ā Apply to purchase retirement service credit, as permitted by law, in any of New York state's public retirement systems.
In Plan Roth ConversionĀ (PDF) ā Provides instructions on how to convert your pre-tax account to a Roth account within the Plan. Contact your Account Executive or call the HELPLINE to get a copy of the In Plan Roth Conversion form.
Transfer Assets into the Plan
Incoming Rollover (PDF) ā Roll money over from other retirement savings accounts into the Plan.
Deferral Change ā Change your deferral percentage. This can also be done online, or by contacting an Account Executive.
Internal Plan to Plan Transfer (PDF) ā Transfer assets between two employers that both participate in the Plan.
Distribution While Employed
Small Inactive Account Withdrawal (PDF) ā Get the qualifications for a withdrawal from a Small Inactive Account.
Self-Certification Unforeseeable Emergency Distribution (PDF) ā Apply for a distribution that you self-certify meets the criteria for an unforeseeable emergency.
HEART Act Form (PDF) ā Allows reservists called to active duty to take distributions from their retirement savings accounts without the usual 10 percent federal tax penalty assessed on early withdrawals.
Distribution After Leaving Employment
Benefit Distribution Request (PDF) ā Start the process needed to request a distribution from the Plan. Fill out this form and submit it via mail for processing. Learn more about how to start receiving income in the Learning Center.
2024 W-4P Federal Tax Form (PDF) - For use when requesting Federal tax withholding above the standard amount for systematic payments lasting for more than 10 years.
2024 W-4R Federal Tax Form (PDF) - For use when requesting Federal tax withholding above the standard amount for the following payment types: one-time payments, systematic payments lasting less than 10 years, Required Minimum Distributions, Unforeseeable Emergency requests, etc.
Tax Withholding Change Request (PDF) ā Change your federal and/or state income tax withholding.
Uniform Life Table (PDF) ā Determine the maximum amount of time during which benefit payments may be made to participants.
Joint and Last Survivor Life Expectancy Table ā Determine the maximum amount of time during which benefit payments may be made to them when the participant has designated their spouse as their sole beneficiary and their spouse is more than ten years younger than they are. Access Table II in the Appendix of this IRS Web page for the Joint and Last Survivor Life Expectancy calculations.
Special Tax Notice (PDF) ā Learn what you need to do before you decide how to receive your Plan benefits.
Required Minimum Distribution (PDF) ā Learn what you need to do upon reaching age 72 and get the necessary paperwork to request a distribution.
Public Safety Officer Insurance Premium Payment Authorization (PDF) ā Allows some public safety officers to request payments from the Plan to directly pay health and longāterm care premiums on a taxāfree basis.
Alternate Payee/Beneficiary
Alternate Payee Benefit Distribution Form (PDF) ā Allows alternate payees to request distribution after a divorce or other court ordered domestic settlement. Read more about how funds are transferred and distributed in the Learning Center.
Beneficiary Distribution Claim (PDF) ā Provides instructions for a beneficiary to make a withdrawal from the Plan upon the death of a participant.
Rollover Distributions
Outgoing Rollover Transfer Kit (PDF) ā This form allows eligible participants to transfer (rollover) their account or a portion of their account to another eligible retirement plan, including an IRA.
Direct Rollover to a Roth IRA (PDF) ā This form allows eligible participants to roll their account or a portion of their account to a Roth IRA.
