HEART Act Form – Allows reservists called to active duty to take distributions from their retirement savings accounts without the usual 10 percent federal tax penalty assessed on early withdrawals.
After Leaving Employment
Benefit Distribution Request – Start the process needed to request a distribution from the Plan. Fill out this form and submit it via mail for processing. Learn more about how to start receiving income in the Learning Center.
2025 W-4P Federal Tax Form and Instructions – For use when requesting Federal tax withholding above the standard amount for systematic payments lasting for more than 10 years.
2025 W-4R – For use when requesting Federal tax withholding above the standard amount for the following payment types: one-time payments, systematic payments lasting less than 10 years, Required Minimum Distributions, Unforeseeable Emergency requests, etc.
Tax Withholding Change Request – Change your federal and/or state income tax withholding.
Single Life Expectancy Table – For use by account holders who are unmarried; married and whose spouse is not more than 10 years younger; or married and whose spouse is not the sole beneficiary of the account.
Joint and Last Survivor Life Expectancy Table – Determine the maximum amount of time during which benefit payments may be made to them when the participant has designated their spouse as their sole beneficiary and their spouse is more than ten years younger than they are. Special Tax Notice – Learn what you need to do before you decide how to receive your Plan benefits.
Required Minimum Distribution – Learn what you need to do upon reaching age 73 and get the necessary paperwork to request a distribution.
Public Safety Officer Insurance Premium Payment Authorization – Allows some public safety officers to request payments from the Plan to directly pay health and long–term care premiums on a tax– free basis.
Alternate Payee/Beneficiary
Alternate Payee Benefit Distribution Form – Allows alternate payees to request distribution after a divorce or other court ordered domestic settlement. Read more about how funds are transferred and distributed in the Learning Center.
Beneficiary Distribution Claim – Provides instructions for a beneficiary to make a withdrawal from the Plan upon the death of a participant.
Rollover Distributions
Outgoing Rollover Transfer Kit – Learn how to transfer assets from the Plan to another eligible retirement plan, including an IRA.
Direct Rollover to a Roth IRA – This form allows eligible participants to roll their account or a portion of their account to a Roth IRA.
